<p><strong>Principal Care Management (PCM) Care Coordinator</strong></p> <p><strong>&nbsp;</strong></p> <p><strong>POSITION DESCRIPTION:&nbsp;</strong></p> <p>MTC Care is the nation's leading telemedicine provider of behavioral health care to skilled nursing, assisted living and independent living facilities seeks, an experienced full time PCM Care Coordinator.</p> <p>The Care Coordinator is to perform remote monitoring and principal care management for patients under the supervision of the medical team.</p> <p>This is a remote position that requires ongoing tracking and monitoring of patient data communication across members of the medical team and engagement with patients and family members reporting out clinical summaries.</p> <p>&nbsp;</p> <p><strong>ESSENTIAL FUNCTIONS:</strong></p> <ul> <li>Assist clinicians with gathering medical information for patients in-between visits utilizing different electronic health record systems.</li> <li>Report any medical concerns monthly for patients such as falls, medication, behavioral concerns and recent hospitalizations.</li> <li>Completion of PCM as assigned and as needed to ensure all interventions are completed each month with approximately 15 patients per day.</li> <li>Report any issues directly to the ancillary program manager and/or Chief Clinical Officer when applicable.</li> <li>Aid with additional employee trainings as necessary/provide support to newer employees that may need additional assistance via "shadowing."</li> </ul> <p><strong>KNOWLEDGE, SKILLS AND ABILITIES:</strong></p> <ul> <li>High level of organizational skills and proven follow up ability</li> <li>Requires both basic knowledge of medical terminology, attention to detail, and excellent written and verbal communication skills</li> <li>Thorough understanding of healthcare technologies including but, not limited to:&nbsp; using Electronic Health Record Systems, Microsoft Outlook, Microsoft Excel and internet/cloud based applications</li> <li>Must be able to communicate with facility staff, colleagues, and medical team members effectively and professionally.&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;</li> </ul> <p><strong>&nbsp;</strong></p> <p><strong>EDUCATION, LICENSING, EXPERIENCE:</strong></p> <p>Minimum High School Diploma required</p> <p>Must have quiet space free of distractions with internet at home for remote setting</p> <p><strong>&nbsp;</strong></p> <p><strong>&nbsp;</strong></p> <p>This job description is not all inclusive and there may be other tasks and responsibilities that you will perform as required.</p> <p>&nbsp;</p><br/><br/>Please mention the word **EARNESTLY** and tag RMzEuMjE3LjM1LjI1NQ== when applying to show you read the job post completely (#RMzEuMjE3LjM1LjI1NQ==).
